VETERINARY CONSENT FORM
Legal Requirement
Clinical Canine Massage – Warkworth acknowledges and respects the Veterinary Surgeons Act 1966 and Veterinary surgeons (exception) Order 2015 by NEVER working on a dog without first gaining prior veterinary consent.
This ensures that in the opinion of the dogs own vet your dog is fit enough to massage and that there are no contraindications to treatment.
Any therapist who works on a dog without veterinary consent is doing so unlawfully and your dog could be at risk!
This form will need to be signed by your vet PRIOR to the initial session.
